While we do not provide direct support services for older adults, people with disabilities, or carers, Wyndham City Council is supported by a strong network of local providers offering these services to our community.

HACC-PYP (Home and Community Care Program for Younger People)

HACC-PYP supports younger people with disabilities, and their carers, to remain independent and connected within their community. This program provides basic support services such as personal care, domestic assistance, and social support.

Eligibility:

  • HACC-PYP is for people aged under 65 (or under 50 for Aboriginal and Torres Strait Islander peoples) who are not eligible for the NDIS.

Our Community Connectors provide personalised guidance to help residents and carers access the services, groups, and activities they need to stay living independently, active, and connected in the community. We are an access point for Wyndham residents to connect with both formal and informal supports tailored to individual needs.
