Wyndham City provides a number of services that aim to promote and maximise the independence and autonomy of frail, older people and people living with a disability in the City of Wyndham. The program includes a suite of tailored support services targeted at enabling residents to continue living safely in their own homes for as long as is reasonably practical.
Our team is committed to delivering timely, equitable, sustainable, high quality support services that focus on identified needs and individual goals. We focus on continually improving the services that we provide and endeavour to ensure that all our resources, services and facilities are responsive, relevant and accessible to all who are eligible to access them.
The services available through the program include In-home support (assistance with Domestic and Personal Care, Respite Services, Minor Home Maintenance, and Delivered Meals), support in accessing local Support Groups and provision of a range of Transport options.
Residents under 65 (Eligibility & Access)
Residents living in Wyndham who are under 65 (or over 50 for a person of Aboriginal or Torres Strait Islander descent) who are living with a disability or chronic health condition and requiring assistance to continue to live independently at home and in the community may be eligible for assistance.

Residents over 65 (Eligibility & Access)
Residents living in Wyndham who are over 65 (or over 50 for a person of Aboriginal or Torres Strait Islander descent) who need assistance with daily living to continue living independently at home and in the community may be eligible for assistance.
