The State Government has appointed mecwacare to take over the delivery of State-funded community care (personal care, domestic assistance and respite), delivered meals, social support, property maintenance and assessment under the Home and Community Care Program for Younger People program from 1 January 2025.

What is changing?

The State Government has appointed mecwacare to take over the delivery of State-funded community care (personal care, domestic assistance and respite), delivered meals, social support, property maintenance and assessment under the Home and Community Care Program for Younger People program from 1 January 2025.

Council will manage the transition to mecwacare for you. Please call (03) 8734 4514 for more information.

What does this mean for me?

If you currently access any of these services under the Home and Community Care Program for Younger People (HACC PYP) or Support for Carers Program (SCP), Council will manage the transition process to mecwacare for you.

If you haven’t already provided Council with consent to do so, please call (03) 8734 4514.

We are committed to working closely with you and your family to ensure a smooth and seamless transition during the changeover period and beyond.

Meanwhile, Lunch with the Bunch, 121 transport and community transport will no longer be delivered by Wyndham City after 31 December 2024. For information about similar programs, services or options please contact the Community Connections and Care Services Team on (03) 8734 4514.

Community Connector Program

Wyndham City’s Community Connectors can support you to access the services you need to stay living at home, active and connected with your community.

We are an access point for Wyndham residents to connect with personalised support to navigate formal and informal services and groups/activities.

Our connectors can work with all residents and carers of residents living within Wyndham.
